Volume over time
When total volume and duration are known, rate = volume ÷ time. Confirm whether the answer should be mL/h or mL/min.
Dose-based infusions
Identify ordered dose (for example mg/h), available concentration (mg/mL), then solve for mL/h. Keep dose and concentration units aligned.
Weight-based rates
Convert patient weight first, then multiply by the ordered mcg/kg/min (or similar) before converting to an hourly volume rate.
